In 2019, the Patient Centered Primary Care Collaborative rebranded to become the Primary Care Collaborative. The logo mark with an individual at the center of cascading circles reflects PCC's ongoing commitment to person- and family-centered care. The logo is based on an icon I designed years ago to visualize the patient-centered principle of primary care, and thus serves as an evolution of PCC's brand.
